Short-Term Disability
Short-Term Disability (STD) insurance can be paid by either the employer or the employee. It provides a source of income if the coverage person becomes disabled due to a covered injury, illness, or mental health condition. A policyholder's spouse and dependents are not eligible for coverage on the disbaility policy.

Hospital Confinement Indemnity
Hospital Confinement Indemnity helps ease the financial burden of hospital stays due to a covered accident or illness or mental illness facility stays due to a mental illness by providing cash benefits.
Hospital Indemnity coversage includes a wide variety of benefits that relate to a person's hospital or mental illness facility confinement, treatments received while confined, and various other benefits not requiring a confinement.

Aflac Vision Insurance (Supplemental Plan)

Eye exams are not subject to a waiting period, meaning eye exams are payable during the first 30 days of coverage. The Eye Examination Benefit pays for an eye examination performed by an optometrist or ophthalmoogist.

Accident
Aflac's Accident plans help reduce the financial impact of a covered accident by providing cash benefits. Aflac Accident plans are designed to provide policyholders with benefits throughout the different stages of care. Policyholders can submit accidentclaim forms along the way, such as claims for emergency treatment, hospitalization, follow-up visits, and therapy.

- Four levels of coverage which all provide benefits for injuries such as fractures, dislocations, lacerations, concussions, burns, emergency dental work, eye injuries and surgical procedures.

Specified - Disease
The Cancer/Specified Disease plans help with the costs of cancer treatment. The plans offer a broad definition of cancer that is designed to provide multiple benefits.

Lump Sum Critical Illness (LSCI)
Lump Sum Critical Illness (LSCI) is designed to provide a lump sum payment to assit with costs associated with covered critical illness events. The benefits are payable as a lump sum, once per person, per lifetime, excluding subsequent event coverage.

Whole Life
Whole Life Iis often referred to as " permanent insurance" because it never expires, as long as premium payments are maintained. Whole Life insurance builds cash value and may help provide the insured with the "peace of mind" that as long as premium payments are maintained, he or she will always be insured.
Features:
- The policy/certificate can be purchased after tax only.
- Policies/certificates can be transferred from payroll to direct with no change in rates.
- Permanent coverage that builds cash value.
Term Life
Term insurance provides coverage for a specific length of time. If the policy/certificate holder dies during the term, the death benefit is paid in full. For the 20 and 30 year term options, if the policy/certificate holder does not die and the term has been reached, the policy/certificate will either terminate, can be renewed for another term, or can be converted to a Whole Life policy or certificate. The 10 year term options also provides the ability for conversion.

- Policy/certificate holders can take the coverage with them (bank draft or direct bill) if they change jobs or retire.

Juvenile Life
Aflac's Juvenile Term Life insurance policy is designed to provide protection for people ages 14 days through 25 years. Age 14 days means the latter of 14 days or the date realeased from the hospital after birth. Juvenile Term and Whole life offer varying face amounts $10k, $20k, or $30k.
Features:
- Juvenile Life plans are available via payroll deduction and direct billing.
- Grandparents are eligible to purchase Juvenile Life policies for their grandchildren. Additional forms and parent or guardian authorization, and even authorization from the child in some cases, may be required.
- A juvenile Term life policy can convert to a Whole Life policy up to double the face amount on the policy anniversary date after the insured's 25th birthday, only at the end of the term, if requested.
- Whole Life face amount doubles at policy anniversary following 18th birthday with no increase in premium.
